Hospital Home Care

Are you a dynamic, motivated individual with exceptional clinical skills?

UHMB are looking for clinical support worker staff to help us our new service ‘Hospital Home Care’. Our service provides high quality outreach care at home for our patients whilst they are awaiting their permanent care provider to begin.

You must be self-motivated and committed to providing a high quality service and also work as part of a team as this role will work closely with other essential support services enabling the patients to remain in their own home. Following the success of service it now been extended to manage the patient’s end of life care needs at home and provide ‘double up’ (2 carers per visit) care as required.

Duties will include but are not limited to:
• Assisting with personal care
• Providing household services including meal preparation
• Assisting patients to develop and maintain their daily living skills and independence as part of their rehabilitation goals.
• Promptly reporting any changes which may affect the health or wellbeing of a patient and liaising with other care providers to meet the patient’s needs.

You will be required to travel to patients homes – please note having your own transport/the ability to travel is an essential requirement to this role and you will require to have business on your vehicle insurance.

Anyone newly appointed to this trust will be subject to the completion of a satisfactory 6 month probation period (this includes bank staff). This will give the new staff member an opportunity to become familiar with the role and working environment.

Please note this posts is fixed term post until 31 March 2018.

Please note that this post is to support the team in the Lancaster and South Lakes area. This is a full time post of 36 hours per week, with staff working a 12 hour shift pattern

Hours of operation will be from 08:00 – 21:00 / 7 days a week.

Please also make it clear that you hold a UK driving license and are prepared to take out business insurance

Returning to practice can seem a little daunting and this programme aims to support you for a period of 3 – 6 months working in a Heath Care Support Worker role ahead of commencing a return to practice programme. This valuable transition period will provide support to enable you to make right decisions regarding your career and re-introduction to studying through completion of the Care Certificate.

We have 5 places available across our Better Care Together health and care partners in North Lancashire and the South Lakes.

An exciting opportunity has arisen for additional band 3 clinical support workers, to join our team.

You will be enthusiastic, motivated and flexible with an ability to adapt to change in order to meet service demands over a seven day period.

You will support nursing and medical staff with the care of patients in all areas of the department.
Duties will include venepuncture, cannulation, recording clinical observations, ECG’s and bladder scanning, as well as general clinical support worker duties.

The department is committed to appraisals, supporting continuous education and professional development.

You will be required to participate in providing 24 hour cover including weekend and night duties.
